Job summary

Bayshore HealthCare is seeking a Field Nurse Case Manager to support oncology patients through enrolment, reimbursement processes, and case management within the Oncology Patient Support Program. You will liaise with physicians, insurance, and regulators to ensure timely access to prescribed treatments.

Based in Montreal, you will travel to physician offices, maintain accurate records, and collaborate with cross-functional teams to improve services and patient outcomes.

Qualifications

• Minimum 2 years of experience in patient support or related programs.
• Bilingual in French and English (oral and written) required.
• Experience with reimbursement processes including provincial/private coverage.
• Familiarity with data privacy laws (PIPEDA) and confidentiality.
• Valid driver’s license with few infractions in past 2 years.

Responsibilities

• Assist prescribing physicians with patient enrolment and on-site visits to gather information.
• Liaise with third-party providers and provincial governments on coverage options.
• Review patient charts and maintain electronic records.
• Schedule and coordinate blood work and notify physicians of results.
• Provide Welcome Call and ongoing communication with physicians and care teams.
• Advocate for patient coverage and assist with documentation and reporting.
• Support on-site physician needs and training for patients and physicians on product administration.
