Job summary

McKesson Canada is seeking bilingual Field Case Manager(s) to join a growing Patient Support Program, supporting patients living with chronic and complex conditions.

You will serve as a primary contact for patients, support onboarding and access-to-treatment activities, provide disease education, coordinate with clinics and healthcare providers, travel within the assigned territories, and maintain accurate documentation in program systems.

Qualifications

• Bilingual in French and English, both written and verbal.
• Experience in a Patient Support Program (PSP) environment preferred.
• Ability to manage multiple priorities in a dynamic setting.
• Strong organizational and relationship-building skills.

Responsibilities

• Serve as a primary point of contact for patients enrolled in the PSP.
• Support patient onboarding, enrollment, and access-to-treatment activities.
• Provide education regarding disease state, treatment expectations, and adherence.
• Assist patients throughout their treatment journey and identify barriers to access.
• Document patient assessments to support continuity of care.
• Collaborate with reimbursement specialists, clinics, and healthcare providers to facilitate access to therapies.
• Support access initiatives and review information required for reimbursement.
• Monitor patient progress and escalate concerns as appropriate.
• Build and maintain relationships with physicians, nurses, clinic staff, pharmacists, and other healthcare stakeholders.
• Provide program updates and patient information to providers in line with privacy rules.
• Maintain accurate documentation within program systems.
• Participate in program meetings, improvement initiatives, and process optimization.
• Ensure activities comply with policies and regulatory requirements.
