Job summary

Main Street Project (MSP) is Winnipeg’s community health centre focused on housing-first and harm-reduction. The Housing Case Manager supports people experiencing homelessness or at risk, coordinating housing, income and health services.

The role emphasizes trauma-informed practice and collaboration with external partners to help participants access stable housing and primary care. The position is full-time until March 31, 2027, based at 637 Main Street, with daytime Monday–Friday hours.

Qualifications

  • Bachelor's degree in a relevant field and frontline experience considered in lieu of a degree.
  • Knowledge of case management principles: eligibility, assessment, planning, implementation, evaluation and documentation.
  • Experience developing care plans that promote positive health outcomes.
  • Experience working with Indigenous communities and culturally diverse populations.

Responsibilities

  • Provide on-going case management support to assess progress and ensure treatment plans.
  • Develop and coordinate individualized care plans with participants and external partners.
  • Assess unsheltered individuals' needs and identify barriers to housing and services.
  • Create and maintain professional relationships with community members and organizations.
  • Maintain confidential records and conduct progress notes and reports.
  • Apply ethics of confidentiality, self-determination and informed consent.
  • Coordinate with landlords, service agencies and advocacy on behalf of participants.
