The Case Manager at Trellis Society for Community Impact supports clients in finding and maintaining housing within a harm reduction framework, focusing on youth in the community of their choice.
They manage a caseload, address housing, basic needs, medical appointments, and connect clients with mental health services, addiction counseling, and system supports while advocating for family reunification and community resources.

Further, Case Managers aim to reduce barriers for their clients, including those of housing, basic needs, medical appointments, and advocating for system supports, mental health services and addiction counseling. Case Managers have an assigned caseload for which they manage and support.
This position reports to a Trellis Program Manager.
