Job summary

Main Street Project in Winnipeg, MB is seeking an Housing Case Manager to support participants through housing-first, harm-reduction case management. You will assess needs, develop individualized care plans, connect participants to primary care, income, and mental health resources, and advocate for access to services.

The role emphasizes trauma-informed care, cultural safety, and collaboration with external agencies, landlords, and Indigenous communities to help participants sustain housing and

Qualifications

  • Bachelor's Degree in a relevant field. Experience frontline with unsheltered preferred.
  • Demonstrated knowledge of case management principles—eligibility, assessment, planning, implementation, evaluating, transitioning.
  • Experience developing care plans that promote positive health outcomes.
  • Experience with Indigenous organizations and culturally diverse populations.

Responsibilities

  • Provide ongoing case management support to assess progress and ensure treatment plans.
  • Develop individualized care plans addressing short- and long-term goals with participants and partners.
  • Assess unsheltered individuals’ needs and identify barriers to housing; develop strategies to meet goals.
  • Build professional relationships with community members and maintain records per protocols.
  • Attend program meetings and coordinate with landlords and service agencies.
