Job summary

Phoenix Society is seeking a dedicated Case Manager to support residents in transitioning to independent living within the community. The role emphasizes case management, orientation, assessment, counselling, and connecting clients to services through collaboration with addiction and employment counsellors.

Responsibilities include group facilitation, crisis intervention, and coordinating with on-site teams and community partners to support recovery, housing, and social integration for clients

Qualifications

  • Post-secondary diploma in case management, social work, psychology or related field.
  • 1–3 years of case management or similar community support experience preferred.
  • Satisfactory criminal records check with vulnerable sector screening.
  • Ability to work in a trauma-informed, client-centered environment.

Responsibilities

  • Provide case management services to an assigned caseload.
  • Observe and assess for emergencies; report and plan safety measures.
  • Maintain and update client files with monthly reporting.
  • Offer supportive counselling and group facilitation to clients and families.
  • Coordinate with medical and mental health teams; liaise with community resources.
  • Attend interdisciplinary team meetings and contribute to discharge planning.
