Job summary

VHA Home HealthCare is seeking a Care Team Scheduling Coordinator for a permanent part-time role in Toronto. You will manage client offers, schedule caregivers, and coordinate service plans to ensure a client-centered delivery of care.

The role requires strong communication with clients, caregivers, and funders, plus knowledge of GTA routes and scheduling systems. Training will be provided, including 3 weeks of in-person onboarding starting Nov 30, 2026.

Qualifications

• Post-secondary certificate/diploma in healthcare education preferred.
• Minimum of three years related work experience in health care or in an administrative scheduling role.
• Excellent interpersonal and telephone skills for professional client interactions.
• Knowledge of GTA and TTC routes or relevant contracted areas.

Responsibilities

• Acceptance of new client offers.
• Scheduling, service changes and updates using scheduling software.
• Maintenance of service plans.
• Regular communications with clients, caregivers, and funders.
