Placements24 is seeking a highly organized Case Manager to join a remote social services team serving clients across Northern Cape and nationally. You will assess client needs, develop individualized plans, and connect individuals and families with housing, employment, healthcare, and financial resources.

You will advocate for clients, maintain confidential records, and use digital systems to track progress while managing a remote caseload.

Qualifications

• Bachelor's degree in Social Work, Human Services, or a related field.
• Minimum of 2 years of experience in case management or social services.
• Strong understanding of social service systems, community resources, and relevant legislation.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency in using case management software and virtual communication tools.
• Ability to work independently and manage a remote caseload effectively.
