Job summary

Placements24 in Kimberley is seeking an experienced Case Manager to join our Social Services team. You will assess client needs, develop individualized support plans, and coordinate services to help clients reach personal and social goals.

This hybrid role blends in-person client work with remote administration, requiring strong communication, advocacy, and collaboration with community partners. A Bachelor’s degree in Social Work or a related field and at least three years of case management

Qualifications

Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
Minimum of 3 years of experience in case management or social services.
Solid understanding of social welfare systems, community resources, and relevant legislation.
Excellent assessment, planning, communication, and interpersonal skills.

Responsibilities

Conduct comprehensive client assessments to identify needs, strengths, and barriers.
Develop, implement, and monitor individualized case plans in collaboration with clients.
Coordinate access to a range of social services, including housing, employment, healthcare, and financial assistance.
Provide ongoing support, counseling, and advocacy for clients throughout their case journey.
Maintain accurate, detailed, and confidential client records and progress notes.
Liaise with community partners, government agencies, and other stakeholders to ensure integrated service delivery.
Conduct regular follow-ups to evaluate client progress and adjust support plans as needed.
