Job summary

CMHA Toronto's Patterson House program is seeking a Case Manager to facilitate placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

Reporting to the Program Manager, the Case Manager will coordinate outreach, assessment, and client-centered services, fostering partnerships with landlords and City of Toronto staff to ensure stable housing outcomes.

Qualifications

• Minimum undergraduate degree in Social Work/Social Science or related field; college diploma with directly related work/life experience will be considered.
• At least 3 years full-time experience delivering direct services to adults with serious mental health issues.
• Knowledge of mental health, criminal justice, concurrent disorders and supportive housing preferred.
• Valid Ontario driver's license with access to a car for workplace use.

Responsibilities

• Outreach and assessment with clients in the community.
• Orientation to program and agency.
• Client-centered engagement, service provision and interventions.
• Skill development and goal planning for clients.
• Providing resources, linkages and advocacy.
• Liaising with landlords and City of Toronto staff.
• Face-to-face visits to clients in the community.
• COVID-19 related tasks and responsibilities as needed.
