Job summary

Haven Toronto in Toronto, Ontario, seeks a part-time Case Manager to provide client-centered support for individuals experiencing homelessness, substance use, and mental health challenges. You will coordinate care, help clients access services, and develop individualized service plans within a multidisciplinary team.

Qualifications

  • Bachelor’s Degree in Social Work (BSW) from an accredited institution.
  • Registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  • Demonstrated experience in case management, assessment, and working with individuals experiencing mental health and/or substance use challenges.

Responsibilities

  • Manage a caseload of clients with complex needs, providing ongoing case management and support.
  • Complete assessments and service planning in collaboration with clients.
  • Provide supportive counselling, brief interventions, and crisis response as needed.
  • Facilitate groups (e.g., life skills, harm reduction, wellness).
  • Support access to housing, income supports, healthcare, and community resources.
  • Maintain accurate and timely documentation on EMHware including case notes and service data.
  • Build and maintain relationships with community partners and service providers.
  • Advocate for clients within systems and support navigation of services.
  • Contribute to a safe, welcoming, and inclusive program environment.
  • Assist clients in accessing core services such as showers, laundry, meals, clothing, and recreation areas.
  • Provide crisis intervention in accordance with Haven Toronto and SMH policies and procedures; document actions taken and recommendations for follow-up.
  • Assist at the front desk by greeting clients, answering calls, registering clients for the program, booking appointments, providing referrals, and offering client support.
  • Administer first aid or call emergency services when required.
  • Work collaboratively with staff who work during the week in order to provide wrap around care.

Case Manager – Haven Toronto
Position Type: Part-Time, Permanent
Start Date: June 22, 2026
Schedule: 8am-4pm, Saturday and Sunday
Reporting to: Manager of Life Enrichment Programs
Compensation: $21,763.64/annum


This position is part of the CUPE [Local # 2289-05] bargaining unit and is subject to the terms
and conditions outlined in the collective agreement.


Since 1933, Haven Toronto has been a drop-in centre where elderly homeless, marginally
housed and socially isolated men can feel safe, where they can form and cultivate friendships,
and become part of a community. Our vision is a safe and just community in which elder
homeless men can live a quality and dignified life.


Summary: The Case Manager provides client-centered support to individuals experiencing
homelessness, substance use, mental health challenges, and complex social needs. Working as
part of a multidisciplinary team, the Case Manager is responsible for coordinating care,
supporting access to services, and helping clients develop and achieve their goals through
individualized service planning.

Knowledge, Skills, and Abilities
  • Strong understanding of substance use, mental health, trauma, and recovery
  • Knowledge of harm reduction and housing-first approaches
  • Understanding of the social determinants of health and barriers faced by individuals
    experiencing homelessness
  • Demonstrated skills in assessment, case management, and supportive counselling
  • Ability to work collaboratively within interdisciplinary teams and with community
    partners
  • Experience working within a trauma-informed and anti-oppression framework
  • Strong interpersonal, communication, and documentation skills
  • Ability to manage competing priorities in a fast-paced environment
Qualifications
  • Bachelor’s Degree in Social Work (BSW) from an accredited institution.

  • Demonstrated experience in case management, assessment, and working with
    individuals experiencing mental health and/or substance use challenges.

  • Additional language skills are an asset
