Job summary

McMan Calgary & Area is seeking a Case Manager to provide individualized, participant-centered support to youth and young adults, helping them navigate housing stability and transition to independence. Working from recovery-oriented and trauma-informed approaches, you will deliver outreach, intensive case management, and system navigation to address barriers to housing, mental health, and well-being.

This role requires on-call rotation and flexible early mornings/evenings.

Qualifications

  • Degree or diploma in Human Services
  • Knowledge of youth homelessness and applicable community resources
  • Experience working with youth in case management
  • Excellent organizational and communication skills
  • Strong writing, organizational, program planning and interpersonal skills
  • Demonstrated interest and knowledge of Indigenous history, language and culture
  • Current Police and Vulnerable Sector Search (within past 6 months)
  • Current Child Intervention check (IRC) (within past 6 months)
  • Current Standard First Aid Certification (within past 3 years)
  • Valid Driver’s license and adequate automobile insurance (2 million dollars + 3rd party liability)
  • 3 years of driving experience with a Class 5 license
  • Suicide Intervention Awareness Training

Responsibilities

  • Manage an assigned case load of 8 youth, scheduling time to meet with each youth weekly and admin time
  • Assist youth in developing and implementing Individualized goals
  • Complete intakes, quarterly reports, case notes and other admin duties in a thorough and timely manner
  • Provide comprehensive community resource information to youth and their natural supports on individual needs
  • Teach and model skills that promote greater self-efficacy
  • Transport youth in personal vehicle to appointments in the community
  • Complete monthly unit checks
  • Utilize a Natural Supports approach to enhance clients' support networks
  • Provide support and resources in the areas of recovery, mental and physical health, education and employment and basic needs
  • Support the organization by participating in a 7 day on-call rotation
