Job summary

CMHA Toronto's Patterson House program is seeking a Case Manager to facilitate placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

Reporting to the Program Manager, the Case Manager will coordinate outreach, assessment, and client-centered services, fostering partnerships with landlords and City of Toronto staff to ensure stable housing outcomes.

Qualifications

• Minimum undergraduate degree in Social Work/Social Science or related field; college diploma with directly related work/life experience will be considered.
• At least 3 years full-time experience delivering direct services to adults with serious mental health issues.
• Knowledge of mental health, criminal justice, concurrent disorders and supportive housing preferred.
• Valid Ontario driver's license with access to a car for workplace use.

Case Manager – Patterson House Program (Full Time – Permanent)

The Role and Opportunity

CMHA Toronto’s Patterson House program is seeking a Case Manager. The Patterson House Program is delivered in partnership with the City of Toronto and include transitional and permanent housing for individuals who have experienced homelessness. Reporting to the Program Manager, the Case Manager will facilitate the placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

SKILLS YOU BRING

• Minimum undergraduate degree in Social Work/Social Science or related field; college diploma with directly related work/life experience will be considered.
• A minimum of 3 years full-time experience in direct service delivery to adults with serious and complex mental health issues, including those with concurrent disorders.
• Knowledge of mental health, the criminal justice system, concurrent disorders, psychosocial rehabilitation, recovery principles and supportive housing strongly preferred.
• Strong assessment, service planning and crisis prevention/intervention skills.
• Excellent communication skills (written and oral) and interpersonal skills.
• Ability to demonstrate an anti-oppression approach to practice is required.
• A valid Ontario driver’s license is required. The successful applicant must have access to a car and insurance for workplace use.

Hiring Range: $54,943 – $60,203
