CMHA Toronto's Patterson House program is seeking a Case Manager to facilitate placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.
Reporting to the Program Manager, the Case Manager will coordinate outreach, assessment, and client-centered services, fostering partnerships with landlords and City of Toronto staff to ensure stable housing outcomes.
CMHA Toronto’s Patterson House program is seeking a Case Manager. The Patterson House Program is delivered in partnership with the City of Toronto and include transitional and permanent housing for individuals who have experienced homelessness. Reporting to the Program Manager, the Case Manager will facilitate the placement and support of Patterson House clients into transitional and supportive housing within a 24/7 staffed CMHA operated building.

Hiring Range: $54,943 – $60,203
