Job summary

Phoenix Society is seeking an Addictions Support Worker for casual on-call shifts at the Phoenix Center (Main Campus). The role supports clients facing mental health and substance use challenges, providing non-clinical care, daily living assistance, and community-based resources.

Responsibilities include case planning, group support, trauma-informed practices, and collaboration with a multidisciplinary team to promote recovery and independent living in the community.

Qualifications

• Diploma in Community Social Service (or equivalent).
• 2 years ofexperience providing one-to-one and group support in community social services.
• Equivalency of education/training/experience accepted.

Responsibilities

• Participates in case planning with clients and healthcare providers.
• Provides individual support and behavior management to clients and families.
• Facilitates psycho-educational workshops.
• Identifies community resources and connects clients to services.
• Responds to critical issues per clinical guidance.
• Maintains client records and progress reports.
