Qualifications

• Diploma in Community Social Service or equivalent required.
• Experience with marginalized populations, substance use and mental health services is beneficial.
• Criminal records check, TB screening, immunizations, and NVCI training may be required.

Responsibilities

• Participates in case planning with clients and care providers.
• Provides individual support to develop life skills and independence.
• Facilitates Psycho-Educational workshops on mental health and life skills.
• Connects clients to community resources and services.
• Documents care plans and progress notes.
