Job summary

VHA Home HealthCare in Ontario is seeking a Physiotherapist to provide consultation, assessment and planned interventions for diverse client groups across Southern Ontario and the Capital region, referred through VHA Rehab Solutions. You will work within a continuous quality improvement framework, promote best-practice care, and coordinate with the client’s care team to support independent living at home.

Qualifications include a Baccalaureate or Master’s in Physiotherapy, registration with the

• Baccalaureate or Masters degree in Physiotherapy from a recognized school.
• Registered with the College of Physiotherapists of Ontario.
• Minimum of 2 years clinical experience, preferably in the community.
• Specialized courses to support areas of clinical expertise.
• Knowledge of community care requirements and resources.
• Excellent consultation and teaching skills with evidence-based practice.
• Familiar with valid and reliable outcome measures.

Responsibilities

• Assesses clients using interviews, standardized and informal assessment techniques.
• Implements client interventions based on assessment information and care plan.
• Ensures services are best practice and evaluated through outcome measures.
• Attends client related meetings to report on assessment findings and plans.
• Ensures coordinated delivery of care through ongoing communication with all members of the client’s team.
• Documents all assessment findings, treatment plans, progress and discharge summaries.
• Prepare statistical records of client care and related activities.
• Adheres to the code of ethics of the profession and standards of practice.
