VHA Home HealthCare in Toronto is seeking a Physiotherapist (PT) for the House Calls Program to deliver in-home care to seniors with cognitive, physical, and social needs. The role focuses on assessment, intervention planning, and ongoing case management within an interdisciplinary team.

Responsibilities

• Assess clients using interviews and standard assessment methods.
• Implement interventions based on assessment and integrated care plan.
• Ensure services are evidence-based and outcome-driven.
• Attend client-related meetings to discuss plans and progress.
• Coordinate delivery of care with all team members.
• Document assessment findings, treatment plans, progress and discharge summaries.
• Prepare statistical records of client care as required.

House Calls delivers comprehensive in-home healthcare to vulnerable seniors who experience barriers accessing traditional office or clinic-based care.A partnership between primary care, SPRINT Senior Care and VHA Home HealthCare, our integrated team includes Physicians, Nurse Practitioners, Social Workers, Occupational Therapists, and Physiotherapists, supported by in-office Coordinators.The House Calls team strives to improve the health and quality of life for all our patients. Our care plans include comprehensive and ongoing primary care, chronic and acute disease management, medication administration, in-home interdisciplinary assessments, ongoing case management, and assistance with navigating the healthcare system. The physiotherapist will work within a continuous quality improvement framework that reflects VHA’s vision and mission of services.
