Job summary

The Care Company is seeking a qualified Physiotherapist to join its community rehabilitation team in Ontario. This casual, flexible position can be structured as an employee or independent contractor, offering autonomy over your own home-care caseload while receiving interdisciplinary support.

Qualifications

• Must be registered with the College of Physiotherapists of Ontario (CPO).
• Experience providing home and community care is strongly preferred.
• Strong assessment and rehabilitation planning skills, with a focus on mobility and aging in place.

Responsibilities

• Perform in-home physiotherapy assessments within referral timelines.
• Develop individualized, evidence-based treatment plans to maximize mobility and safety.
• Provide follow-up visits including therapeutic exercise, gait training, balance retraining, and falls prevention.
• Educate clients and caregivers to promote long-term adherence to home exercise programs.
• Collaborate with interdisciplinary team to support seamless care transitions.
• Maintain accurate and timely clinical documentation.
