Job summary

VHA Home HealthCare in Toronto, ON seeks a part-time Physiotherapist to deliver consultations, assessments and planned interventions for diverse client groups across Southern Ontario and the Capital region. The role emphasizes evidence-based practice, client-centered care and collaboration with the client’s care team.

The successful candidate will hold a physiotherapy degree, be in good standing with the Ontario College of Physiotherapists, and have a minimum of two years of clinical experience

Qualifications

  • Bachelor's or Master's degree in Physiotherapy from a recognized school.
  • Registered, in good standing, with the College of Physiotherapists of Ontario.
  • Minimum of 2 years clinical experience, preferably in the community.
  • Specialized courses to support areas of clinical expertise.
  • Clinical expertise to provide a variety of physiotherapy interventions.
  • Knowledge of community care requirements and resources available.
  • Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.

Responsibilities

  • Assess clients using interviews and standardised assessment techniques in line with professional guidelines.
  • Implement client interventions based on assessment information and care plans.
  • Ensure services provided are best practice and outcome-driven.
  • Attend client-related meetings to report on assessment findings and plans.
  • Coordinate delivery of care through ongoing communication with the clients team.
  • Document assessment findings, treatment plans and discharge summaries as required.
  • Prepare statistical records of client care and related activities as required by VHA and regulators.
  • Adhere to the code of ethics of the profession.

I feel welcome and at home.”View more comments from our clients and their family members.**Pay Range**VHA's Physiotherapist is responsible for providing physiotherapy consultation, assessment and planned interventions for diverse client groups across Southern Ontario and the Capital region, referred through VHA Rehab Solutions and Champlain Rehab Solutions. The physiotherapist will work within a continuous quality improvement framework that reflects VHA’s vision and mission of services.
