Job summary

VHA Home HealthCare in Toronto is seeking a Physiotherapist (PT) for the House Calls Program to deliver in-home care to seniors with cognitive, physical, and social needs. The role focuses on assessment, intervention planning, and ongoing case management within an interdisciplinary team.

You will provide physiotherapy consultation and interventions, participate in quality improvement, and support coordinated care from assessment through discharge, ensuring evidence-based practice and excellent

Qualifications

• Masters degree in Physiotherapy from a recognized school.
• Registered, in good standing, with the College of Physiotherapists of Ontario.

Responsibilities

• Assess clients using interviews and standard assessment methods.
• Implement interventions based on assessment and integrated care plan.
• Ensure services are evidence-based and outcome-driven.
• Attend client-related meetings to discuss plans and progress.
• Coordinate delivery of care with all team members.
• Document assessment findings, treatment plans, progress and discharge summaries.
• Prepare statistical records of client care as required.
