Job summary

VHA Home HealthCare in Toronto is seeking a Physiotherapist for the House Calls program (Adults) to deliver in-home physiotherapy services on a 35-hour work week. You will assess, plan, and implement individualized interventions for seniors living at home, coordinating with a multi-disciplinary team to support independence and safety in the home setting.

Qualifications

  • Masters degree in Physiotherapy from a recognized school of physiotherapy.
  • Registered, in good standing, with the College of Physiotherapists of Ontario.
  • Valid Driver's License with access to a vehicle.
  • Minimum of 1 year clinical experience, preferably in the community.
  • Specialized courses to support areas of clinical expertise.
  • Clinical expertise to provide a variety of physiotherapy interventions.
  • Knowledge of community care requirements and resources available.
  • Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.

Responsibilities

  • Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
  • Implements client interventions based on assessment information, client input and in conjunction with the client's integrated care plan.
  • Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measures.
  • Attends client related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
  • Ensures coordinated delivery of care through ongoing communication with all members of the client's team regarding plans and schedules for interventions.
  • Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
  • Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.

We are seeking Physiotherapist (PT) - House Calls Program (35 hours per week)

House Calls is a Toronto-based interdisciplinary mobile healthcare team that provides care to seniors living at home with cognitive, physical and social frailties. House Calls delivers comprehensive in-home healthcare to vulnerable seniors who experience barriers accessing traditional office or clinic-based care.

A partnership between primary care, SPRINT Senior Care and VHA Home HealthCare, our integrated team includes Physicians, Nurse Practitioners, Social Workers, Occupational Therapists, and Physiotherapists, supported by in-office Coordinators.

The House Calls team strives to improve the health and quality of life for all our patients. Our care plans include comprehensive and ongoing primary care, chronic and acute disease management, medication administration, in-home interdisciplinary assessments, ongoing case management, and assistance with navigating the healthcare system. The House Calls team also facilitates safe discharges and transitions from the emergency department and hospital inpatient beds to enable safe recovery and care in the home setting.

VHA's Physiotherapist is responsible for providing physiotherapy consultation, assessment and planned interventions for diverse client groups living in Toronto. The physiotherapist will work within a continuous quality improvement framework that reflects VHA's vision and mission of services. To promote and maintain good public relations with clients, customers and VHA staff, volunteers and associates. #LI-JO1

Key Areas of Accountability

VHA's Physiotherapist have the following key areas of accountability, but not limited to:
