Qualifications

Responsibilities

• Conduct initial client phone assessment, care planning and goal setting.
• Coordinate in-home services to support clients receiving care at home.
• Manage escalations and support frontline nurses in the community.
• Manage consumer care throughout the journey (on-boarding, referrals, initial care planning, discharge).
• Provide telephone support, education and advice.
• Discharge planning including referrals to long-term services.
• Monitor patient progress, identify risks, and elevate concerns.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Maintain accurate clinical documentation and reporting in line with standards.
