Medibank Private Limited in Australia seeks an experienced Registered Nurse for a full-time, virtual role within the SA Community Care program. You will coordinate care for clients at home, connect patients and providers, and ensure timely, patient-centered service.

Qualifications

• Registered Nurse with current unrestricted AHPRA registration.
• Experience working in an acute setting (community/hospital).
• Strong understanding of care coordination, case management, or complex care (highly desirable).

Responsabilités

• Conduct clients' initial phone assessment, care planning and goal setting.
• Coordinate in-home services to support clients receiving care at home.
• Manage escalations and provide support for frontline nurses in the community.
• Manage consumer care throughout the journey (on-boarding, triaging referrals, initial care planning, discharge).
• Provide telephone support, education and advice.
• Discharge planning including referrals to long-term services.
• Monitor patient progress, identify risks, and elevate concerns.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Maintain accurate clinical documentation and reporting in line with standards.
