Qualifications

• Registered Nurse with current unrestricted AHPRA registration.

Responsibilities

• Conduct clients' initial phone assessment, care planning and goal setting.
• Coordinate in-home services to support clients receiving care at home.
• Manage escalations and provide support for frontline nurses in the community.
• Manage consumer care throughout the journey (on-boarding, triaging referrals, initial care planning, discharge).
• Provide telephone support, education and advice.
• Discharge planning including referrals to long-term services.
• Monitor patient progress, identify risks, and elevate concerns.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Maintain accurate clinical documentation and reporting in line with standards.

Looking to join a team coordinating care and services to support patients in their own home? We have a full-time opportunity available to work within the SA Community Care (SACC) program.

In this virtual role, you’ll sit at the centre of the care journey, connecting patients, families, and providers to ensure care is coordinated, timely, and genuinely patient-centred.

Key Responsibilities

About you

• Digital proficiency and ability to work virtually
• Excellent communication and relationship-building skills
• Confident working independently while collaborating within a multidisciplinary team
• Highly organised with strong clinical judgment and problem-solving skill
