The GEO Group Australia is seeking a Throughcare Case Manager to deliver pre-release planning and post-release support for individuals transitioning from Ravenhall Correctional Centre. The role is based at the Bridge Centre, Richmond, VIC, with travel to Ravenhall as needed.

You will work in a trauma-informed, strengths-based framework to support successful reintegration into the community. You will develop partnerships with internal and external stakeholders, coordinate care plans, and assist

Qualifications

• Experience in case management and coordinating services.
• Understanding of reintegration outcomes for people released from prison.
• Ability to work within trauma-informed frameworks and complex needs.

Responsabilités

• Maintain a caseload with varying reintegration needs and provide outreach.
• Develop post-release plans identifying needs and supports.
• Coordinate pre-release planning for post-release programs (e.g., Nexus, XIT-180).
• Assist clients to implement Individual Reintegration Plans, including housing, education/employment, and health services.
• Provide front-of-house and community reintegration support.
• Document attendance, progress and outcomes to monitor improvement.
• Participate in team meetings, supervision and training to strengthen practice.
