Job summary

Claro supports people with complex needs to live more independent and confident lives. You will work with clients requiring mobility and daily care assistance, coordinating with families and medical professionals.

We seek a warm, patient, and genuinely caring individual who communicates well, adapts to changing shifts, and collaborates effectively with a multidisciplinary team. Induction and ongoing clinical support are provided.

Responsibilities

• Person-centered planning with client, family, and advocates.
• Assist with complex physical care: mobility, transfers, positioning, daily tasks, and devices as per plan.
• Monitor medical needs, follow plans, and liaise with care team when changes occur.
• Provide social time and participation aligned with client interests.
• Support autonomy and advocacy; respect consent and promote independence.

What you'll do (*tailored to each client as required*)

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
