Job summary

Claro Disability Services is seeking a compassionate support role to work with clients with complex needs in Victoria. You’ll coordinate care plans, assist with mobility and daily tasks, and liaise with medical teams to monitor changes in conditions.

You will enable meaningful social participation, support autonomy, respect cultural preferences, and collaborate with families and health professionals to deliver high-quality, person-centered care.

Qualifications

• Person-centered, warm, patient and caring approach.

• Desirable but optional: manual handling, epilepsy management or disability-support training.

Responsibilities

• Engage in person-centered planning with client, family and advocates.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being

What are the hours?

• Monday 5pm-8pm
• Tuesday 4pm-8pm
• Wednesday 5pm-7:30pm
• Friday 5pm-8pm
