Job summary

Claro Disability Services is seeking a dedicated Support Worker for Mount Barker accommodation. You will work closely with clients with complex needs, supporting daily living, mobility, medical tasks, and social participation in a collaborative team.

Responsibilities

• Person-centered planning: discuss goals and routines with clients and families.
• Provide complex physical care including mobility, transfers, positioning and daily tasks as outlined in plans.
• Support medical needs, monitor changes and liaise with care teams as required.
• Facilitate social time and community access aligned with client interests.
• Promote autonomy, advocate for client choices, and respect consent.
• Show cultural humility by respecting backgrounds, beliefs and values.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
