Job summary

Claro Disability Services is seeking a part-time permanent disability support worker in Adelaide to help people with complex needs achieve independence. You’ll provide person-centered planning, mobility and daily tasks, medical supports, and community participation.

You’ll collaborate with families, medical professionals, and a clinical team to tailor supports to each client’s goals, values, and culture, while building trusting relationships and ensuring safety.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
