Job summary

Claro Disability Services is seeking a dedicated Support Worker for Mount Lawley, WA. This part-time role involves working with clients who have complex needs, assisting with mobility, daily tasks, and medical plans, and supporting social participation.

You will collaborate with families and clinical teams to deliver person-centered care, respecting independence and cultural preferences. Training is provided, and shifts vary to fit your life and client needs.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
