Claro Disability Services in Melbourne is seeking compassionate support workers to assist clients with complex needs across mobility, daily routines, and medical tasks.

You will collaborate with families and health professionals, adapt to changing shifts, and help clients achieve independence with person-centered approaches and culturally respectful care.

Mansioni

• Chat with the client (and family/advocates), understand goals and routines.
• Assist with mobility, transfers, positioning, daily tasks, and devices as per plan.
• Monitor changes, follow medical/clinical plans, liaise with care team.
• Provide social time and participation aligned with client interests.
• Support autonomy, respect consent, boost independence where possible.
• Respect backgrounds and culture, practice cultural humility.
• Mealtime management, manual handling, medication administration; PEG desirable.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
• Mealtime Management, Manual Handling, Medication Administration, PEG desirable, Changed Behaviour
