Job summary

Claro Disability Services in Melbourne is seeking compassionate carers to support clients with complex needs at home. You will assist with mobility, daily tasks, medical plans, and meaningful activities to promote independence.

This role requires flexibility, empathy, and strong teamwork with families and clinical staff. You’ll work with diverse clients, adapting to routines while maintaining safety and dignity.

Responsibilities

• Chat with clients and families to understand goals and routines.
• Assist with mobility, transfers, positioning and daily tasks.
• Follow medical plans and liaise with care teams as needed.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
