Job summary

Claro Disability Services in Camberwell, VIC is seeking caring staff to support clients with complex needs across mobility, medical tasks, daily living, and community participation.

You’ll tailor supports to individual goals, work with families and clinical teams, and build meaningful relationships through flexible schedules and respectful, client-centered care.

Responsibilities

• Chat with client and family to understand goals and routines (person-centered planning).
• Assist with mobility, transfers, daily tasks, and devices per care plan.
• Monitor medical needs, follow plans, and liaise with care team as needed.
• Enable social participation and meaningful activities aligned with interests.
• Support autonomy and advocacy, respecting consent and independence.
• Respect clients' backgrounds, beliefs, and culture, delivering care with humility.

What you’ll do

• Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
• Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
• Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
• Social time and participation: Bring meaning, fun, and community access aligned with interests.
• Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
• Cultural humility: Respect backgrounds, beliefs, and ways of being
• You will be working with clients with stroke, Spina Bifida, Non verbal, complex care ,Stoma care, bladder care, catheter care, bowel care ,meal prep, personal care, medication, community participation, use of electric wheelchair,cleaning, cooking, shopping, client specific management plans.
