Job summary

Access Support Coordination in Rockhampton is seeking a dedicated Support Coordinator to help participants navigate the NDIS, understand their options and maximise their funding.

You will build trust, coordinate supports with local providers, gather evidence, advocate for participants, and contribute to a patient-centered, outcomes-driven team. A clear understanding of plan goals, strong communication, and the ability to work collaboratively with families and services are essential.

Qualifications

  • Experience in a Support Coordination role within the disability sector.
  • Qualifications in Disability, Community Services, Social Work or related field.
  • Knowledge of the NDIS processes and how to navigate funding and supports.

  • Helping participants understand and get the most out of their NDIS plans
  • Coordinating supports efficiently to maximise outcomes and funding utilisation
  • Gathering supporting evidence reflecting participants' evolving needs and future funding requirements
  • Identifying opportunities across employment, recreation, education and housing
  • Advocating for participants and liaising with mainstream services to ensure the right supports are in place

You're the Right Person If
  • You have proven experience in a Support Coordination role within the disability sector
  • You hold qualifications in Disability, Community Services, Social Work or a related field
  • You have a strong knowledge of the NDIS and its processes
  • You're able to manage KPIs and billable hour targets effectively
  • You're a natural connector, excellent with people and confident solving problems with a genuine participant-first approach
  • You hold a current Driver's Licence, Working with Children Check and NDIS Worker Screening Clearance
