Job summary

Concept Care in NSW is seeking an experienced and compassionate Support Coordinator to help participants understand, implement and maximise the benefits of their NDIS plans.

You will deliver high-quality, participant-focused support, build strong relationships, coordinate services, monitor budgets, and contribute to plans reviews, driving positive outcomes.

Qualifications

• Experience in NDIS Support Coordination or case management.
• Strong understanding of NDIS frameworks.
• Excellent communication and relationship-building skills.
• NDIS Worker Screening Check, WWCC, and Police Check.

Responsibilities

• Understand and implement NDIS plans for participants.
• Coordinate services and connect participants with suitable providers.
• Build strong relationships with participants, families, and stakeholders.
• Promote independence, choice and capacity building.
• Monitor budgets and ensure NDIS compliance.
• Prepare reports and support plan reviews.

We're looking for an experienced and compassionate Support Coordinator who is committed to achieving positive outcomes for participants and their families.

About the Role

This is a dedicated Support Coordination role focused on helping participants understand, implement and maximise the benefits of their NDIS plans. The role is centred on delivering high-quality, participant-focused support, building strong relationships, and assisting participants to achieve their goals and access the services they need.

What You'll Be Doing

• Supporting participants to understand and implement their NDIS plans
• Coordinating services and connecting participants with suitable providers
• Building strong relationships with participants, families, and stakeholders

What You'll Bring

• Experience in NDIS Support Coordination and/or case management
• Strong understanding of the NDIS and relevant frameworks
• Excellent communication and relationship-building skills
• Strong organisational and time management abilities
• Qualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)
• Confidence using Microsoft Office and case management systems
• NDIS Worker Screening Check
• Working With Children Check (WWCC)
• National Police Check
• Current Driver's Licence and reliable vehicle
