Job summary

Latrobe Community Health Services in Morwell, VIC is seeking a compassionate Social Support Program Leader to plan, develop and facilitate centre-based and community programs aligned with a Wellness and Reablement approach.

You will support participants to maintain independence and wellbeing, coordinate volunteers and transport, and monitor wellbeing while maintaining records. Requires a Driver's Licence, Working with Children Check, and relevant qualifications.

Located across out Social Support Program Sites, permanent, part time (60 hours per fortnight) position available

Make a real difference in the lives of older adults and vulnerable community members. We're seeking a compassionate and motivated Social Support Program Leader to join our team and help deliver engaging programs that promote independence, social connection and wellbeing.

Reporting to the Team Leader, you will assist in planning, developing and Facilitating centre-based and community programs align with a Wellness and Reablement approach. In this role, you will support participants to maintain their independence and wellbeing by delivering engaging programs and providing individual assistance where required. You will facilitate daily activities, monitor participant wellbeing, coordinate volunteers and transport services, maintain accurate records, and contribute to the continuous improvement of programs and services.

To be successful, you will require;

Victorian Drivers Licence

A current Working with Children Check

Holds or is working towards a Certificate IV or Diploma in Individual Support, Ageing Support and/or Leisure and Health (or willingness to obtain)

Current CPR

Ability to drive a 12-seater social support bus
