Job summary

Latrobe Community Health Services in Morwell, VIC is seeking a compassionate Social Support Program Leader to plan, develop and facilitate centre-based and community programs aligned with a Wellness and Reablement approach.

You will support participants to maintain independence and wellbeing, coordinate volunteers and transport, and monitor wellbeing while maintaining records. Requires a Driver's Licence, Working with Children Check, and relevant qualifications.

Qualifications

• Strong interpersonal, communication and organisational skills.
• Ability to support frail aged and vulnerable clients with sensitivity to cultural diversity.
• Experience planning and facilitating client-focused activities.
• Ability to supervise volunteers and work within a multidisciplinary team.
• Good IT skills, including client reporting systems and Microsoft applications.
