Five Good Friends is seeking a Self-Managed Care Coordinator to empower Members and their Authorised Representatives to self-manage care with confidence. You’ll help navigate funding, use digital tools, and build strong relationships with Helpers while ensuring safety and quality.

Qualifications

• Qualifications in case management, aged care, disability, or human services.
• Experience assessing, planning, and coordinating care for older people or individuals with disabilities.
• Confident using technology and app-based tools.
• Experience in self-managed home care or NDIS supports is highly regarded.

Responsabilités

• Assess needs and develop goal-focused Help Plans for self-managed care.
• Explain funding and the Five Good Friends model to Members and Representatives.
• Action Remote Care Monitoring alerts with timely responses.
• Provide onboarding, app support, and ongoing guidance to Members.
• Match Members with suitable Helpers and support long-term care relationships.
• Monitor wellbeing, coordinate care delivery, and liaise with health professionals.
• Maintain knowledge of Aged Care Standards and internal workflows, ensuring compliance.
• Ensure accurate documentation across systems.
