Job summary

Calvary Home Care in Adelaide is seeking an experienced Registered Nurse to deliver high-quality palliative care at clients' homes and residential settings. You will work independently overnight, manage complex symptom control, educate families and coordinate with the care team.

Applicants must hold current AHPRA registration, a minimum of 2 years in palliative care, and post-graduate studies in Palliative Care; NDIs clearance and police check are required.

Qualifications

• Current AHPRA Nursing registration.
• Minimum 2 years palliative care nursing experience; community experience desirable.
• Postgraduate qualifications (or working towards) in Palliative Care.
• NDIS Clearance (or willingness to obtain).
• Police Clearance with in 6 months (or willingness to obtain).
• Use of own car – KM allowance provided; full driver's licence.
• CPR and Manual handling.

Responsibilities

• Provide care to palliative clients in home or community settings.
• Support and educate families and carers on care plans.
• Administer and prepare s/c medications and CSCI for symptom management.
• Perform wound care, IDC/SPC care and dressings as needed.
• Accurate documentation in client records; update care plans.
• Communicate with clinical team and referrers to coordinate care.
• Develop initial care plans and monitor for deterioration.
• Escalate clinical concerns promptly and participate in quality improvement.

About the role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.

Key Responsibilities

• Support and education for clients with terminal diagnoses
• Education and support for families and carers
• Administration and preparation of s/c medications and CSCI for symptom management
• General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
• Accurate/ Timely documentation in all client records.
• Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
• Complete initial assessments/develop care plans to ensure best outcome for our clients.
• Monitor and update care plans to ensure appropriate care is provided
• Recognise and respond to clinical deterioration, with appropriate escalation.
• Promoting best practice and initiating quality improvement activities
