Calvary Home Care - Adelaide is seeking an experienced Registered Nurse to deliver high-quality palliative care in clients' homes, including overnight support and education for clients and families. The role focuses on symptom management, End of Life care, and coordinating care in home or residential settings.

The ideal candidate holds current AHPRA registration with 2+ years in palliative/community care, postgraduate study in palliative care, and a willingness to undertake NDIS and police

Qualificações

• Current AHPRA Nursing registration.
• Minimum 2 years palliative care nursing experience; community experience desirable.
• Postgraduate qualifications (or working towards) in Palliative Care.

Responsabilidades

• Support clients with terminal diagnoses and provide education for families and carers.
• Administer and prepare subcutaneous medications and CSCI for symptom management.
• Deliver general nursing care including wound care and catheter care; maintain accurate documentation.
• Communicate with clinical teams and referrers to ensure quality care.
• Develop and update care plans; monitor for deterioration and escalate when needed.

About the role

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.

Key Responsibilities

• Support and education for clients with terminal diagnoses
• Education and support for families and carers
• Administration and preparation of s/c medications and CSCI for symptom management
• General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
• Accurate/ Timely documentation in all client records.
• Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
• Complete initial assessments/develop care plans to ensure best outcome for our clients.
• Monitor and update care plans to ensure appropriate care is provided
• Recognise and respond to clinical deterioration, with appropriate escalation.
• Promoting best practice and initiating quality improvement activities

What you bring

• Current AHPRA Nursing
• Minimum 2 years palliative care nursing experience, community experience desirable
• Post graduate qualifications (or working towards) in Palliative Care
• NDIS Clearance (or willingness to obtain one)
• Police Clearance with in 6 months (or willingness to obtain one)
• Use of own car – KM allowance provided
• Full drivers licence, reliable and comprehensively insured car
• CPR and Manual handling
