Job summary

Calvary Home Care in Adelaide is seeking an experienced Community Registered Nurse specialising in palliative care to deliver high-quality in-home care for clients across short and long-term programs. The role involves overnight support, complex symptom management, and client education in home or residential settings.

Key responsibilities include care planning, wound care, medication administration, and effective communication with families and the clinical team.

Qualifications

• Current AHPRA Nursing registration is required.
• Minimum 2 years of palliative care nursing experience preferred.
• NDIS clearance or willingness to obtain one is required.
• Police clearance within 6 months is required.
• Owning a car with KM allowance is provided.
• CPR and manual handling competencies expected.

Responsibilities

• Support and educate clients with terminal diagnoses.
• Educate and support families and carers.
• Prepare and administer subcutaneous medications and CSCI for symptom management.
• Provide general nursing care including wound care and catheter dressings.
• Document accurately and timely in client records.
• Communicate within clinical and community palliative care teams and referrers.
• Complete initial assessments and develop care plans for clients.
• Monitor and update care plans to ensure appropriate care.
• Recognise deterioration and escalate as needed.
• Promote best practice and participate in quality improvement.

Community Registered Nurses - Palliative Care

As a member of the Calvary Home Care team and as an experienced RN, you will provide care to palliative care clients from various short term/long terms programs. You will provide complex care to clients by working independently to support clients and their stakeholders overnight. As a Palliative Care nurse, you will provide high quality patient education, symptom management and End of Life Care in the comfort of a client’s home or residential accommodation.

Key Responsibilities

• Support and education for clients with terminal diagnoses
• Education and support for families and carers
• Administration and preparation of s/c medications and CSCI for symptom management
• General nursing care including simple and complex wound care, IDC/SPC care, PICC dressings
• Accurate/ Timely documentation in all client records.
• Appropriate communication within the clinical team and community palliative care teams and referrers to ensure care is provided.
• Complete initial assessments/develop care plans to ensure best outcome for our clients.
• Monitor and update care plans to ensure appropriate care is provided
• Recognise and respond to clinical deterioration, with appropriate escalation.
• Promoting best practice and initiating quality improvement activities

What You Bring

• Current AHPRA Nursing
• Minimum 2 years palliative care nursing experience, community experience desirable
• Post graduate qualifications (or working towards) in Palliative Care
• NDIS Clearance (or willingness to obtain one)
• Police Clearance with in 6 months (or willingness to obtain one)
• Use of own car – KM allowance provided
• Full drivers licence, reliable and comprehensively insured car
• CPR and Manual handling
