Job summary

Calvary Community Care in Albury NSW is seeking a Registered Nurse to deliver quality community health services. You will assess client needs with a multidisciplinary team, plan and coordinate nursing care, and support clients in achieving optimal health outcomes within Home Care, Community Nursing and associated services.

You will work closely with case managers and families, bringing clinical expertise, strong communication, and a patient-centred approach to a supportive team environment.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• First Aid certificate current.
• Recent nursing/clinical experience in the last 5 years or evidence of a refresher course.
• Strong communication, interpersonal and organisational skills.

Responsibilities

• Work with clients and families to identify and achieve care goals.
• Develop and maintain comprehensive assessments to ensure client safety and care needs are met.
• Coordinate the provision of nursing care, including assessments, wound management, and liaison with Case Managers and the multidisciplinary healthcare team.

About the role

As a member of the Calvary Community Care team, you will deliver quality services to meet the needs of our clients and contribute to a positive experience whilst at Calvary Community Care. Assist in planning, assessing and evaluating our clients needs with the help of our multidisciplinary team, to promote positive health outcomes.The Registered Nurse provides clinical support to our clients and collaborates with our support workers to deliver exceptional care to our clients in the community.Located in Albury and surrounding area.

Key Responsibilities

• Work with clients and families to identify and achieve care goals.
• Develop and maintain comprehensive assessments to ensure client safety and care needs are met.
• Coordinate the provision of nursing care, including assessments, wound management, and liaison with Case Managers and the multidisciplinary healthcare team.

About You

• AHPRA Registration and current First Aid Certificate
• Recent nursing/clinical experience in the last 5 years or evidence of a refresher course
• Effective communication, interpersonal and organisational skills
• Initiative and the desire to continually improve client care
• Capability to prioritise tasks and achieve goals within set timeframes
• Candidates with an understanding of the community nursing sector - Home Care Packages, Department of Veteran Affairs and Commonwealth Home Support Program will be considered favourably
• A desire to join a team committed to the care of others in a mission based organisation
