Job summary

Fresh Hope Communities is seeking an Registered Nurse to provide holistic aged care across North Nowra. The role supports residents with health, wellbeing and community connection in a setting that values Kindness, Integrity, Connection and Optimism.

You will lead care staff, manage the nursing process, and collaborate with multidisciplinary teams to ensure person-centred care and high-quality outcomes for residents and families.

Qualifications

• Aged care nursing experience is essential.
• Minimum of 3 years of experience as a Registered Nurse.
• Current AHPRA registration and knowledge of aged care regulations.
• Ability to lead and coordinate care staff and maintain standards.

Responsibilities

• Provide the highest standard of care for residents in line with Aged Care Standards and Act.
• Deliver comprehensive care and individual case management per the clinical framework.
• Lead and coordinate care staff to deliver medication administration and activities.
• Coach staff to enhance skills and close knowledge gaps.
• Liaise with multidisciplinary team for holistic, person-centered care.
• Build positive relationships with residents, families, and staff.

Job description

As a Registered Nurse you will deliver holistic care for residents that prioritizes health, wellbeing, and community connection to support the changing needs of residents.

Your Contribution

• Provide the highest standard of care for residents in accordance with the Aged Care Standards and Aged Care Act
• Deliver comprehensive care and individual case management to residents according to the organizational clinical framework and care model.
• Ensure effective delivery of the nursing process through the effective resident entry, assessment, care planning and delivery, evaluation and exit recognized by professional standards.
• Lead and coordinate care staff to effectively deliver residents care, everyday activities, administration of medication and interventions that promote overall resident well-being and meaningful engagement
• Coach the care staff to enable them to deliver high quality care, develop skills and knowledge gaps.
• Liaise with a multidisciplinary team to ensure a holistic person-centered approach
• Build positive relationships with Residents, families, and other staff members

The Essentials

• Aged Care nursing experience is essential
• Minimum of 3 years of experience as a Registered Nurse
• Current registration with Australian Health Practitioner Regulation Agency (AHPRA).
• Knowledge of legislation, regulations and evidence based best practice relevant to aged care
