Job summary

Mission Australia is seeking a Recovery Case Worker to support individuals on their recovery journeys within a person-centred framework. You will provide practical support, case management and outreach to empower participants to build independence and improve overall wellbeing.

Qualifications

• Experience supporting people experiencing mental health challenges, psychosocial disability or complex life circumstances.
• Sound understanding of recovery-oriented practice, psychosocial rehabilitation and strengths-based approaches.
• Demonstrated ability to build genuine, respectful relationships and engage people from diverse backgrounds.
• Strong communication skills, with the ability to work collaboratively with participants, families, service providers and community stakeholders.
• Experience developing, implementing and reviewing individualised support or case management plans.
• High level of administrative and organisational skills, including accurate record keeping and report writing.
• Confidence using technology and client management systems, including Microsoft Office applications.
• Ability to work independently, manage competing priorities and respond effectively to changing needs.

Responsibilities

• Provide recovery-oriented case management that supports participants to achieve their goals and improve wellbeing.
• Build positive, trusting relationships that empower participants to develop independence and community connections.
• Complete client intake, assessment and onboarding activities, ensuring accurate documentation and record keeping.
• Develop and review strengths-based case plans tailored to each participant’s needs, goals and aspirations.
• Deliver outreach support and coordinate access to services that contribute to recovery and long-term outcomes.
• Support participants to build daily living skills and successfully transition to independent living or other appropriate supports.
• Maintain accurate case notes, reports and client records in line with organisational and compliance requirements.
• Develop effective partnerships with internal teams, community organisations and external stakeholders to enhance participant outcomes.

Education

Qualification in Community Services, Mental Health, Social Work or a related discipline
