Job summary

Mission Australia is seeking a dedicated Recovery Case Worker to join the Community Living Support team in Goulburn. This part-time role supports people living with severe mental illness to build independence and achieve personal goals through recovery-focused, person-centred care.

You will mentor participants, coordinate services, advocate for self-determination, and collaborate with families, clinicians, and community partners to promote housing, health and social inclusion.

Qualifications

• Qualification in Mental Health, Mental Health Peer Work, Community Services, Social Work, Psychology, Human Services, or related field (or working towards).
• Experience supporting people living with severe and persistent mental illness.
• Familiarity with recovery-oriented and trauma-informed practice.
• Strong ability to build therapeutic relationships and coordinate care.

Responsibilities

• Build positive, professional relationships with participants.
• Deliver recovery-focused psychosocial support tailored to individual goals.
• Facilitate mutual planning sessions and develop individual support plans.
• Provide case management and care coordination for holistic recovery outcomes.
• Support independent living skills and access to community services.
• Deliver psychoeducation and recovery-focused skill development.
• Advocate for participant choice, self-determination and inclusion.
• Maintain case notes, reports, and documentation; coordinate with families and teams.
• Identify risks and respond according to organisational policies.
