Job summary

Mission Australia is seeking a dedicated professional to provide intensive psychosocial recovery support for individuals with complex mental health needs. This on-site role delivers outreach services across the community, helping clients achieve independence and sustainable tenancy outcomes.

You will build strong therapeutic relationships, coordinate supports with external services, and contribute to safer, trauma-informed care in line with individual goals.

Qualifications

• Minimum Certificate IV or higher in Community Services, Mental Health or related field.
• Experience supporting people with complex mental health needs or psychosocial recovery.
• Ability to maintain professional boundaries and sound decisions in complex situations.
• Strong networking and stakeholder engagement across multiple services.

Responsibilities

• Provide intensive psychosocial recovery support to people with complex needs.
• Develop and review person-centred recovery plans and risk assessments.
• Assist with daily living activities and community access to build independence.
• Maintain regular outreach contact in homes and community settings.
• Provide complex case management and connect clients with services.
• Collaborate with tenancy providers and health professionals to support housing stability.
• Complete timely case notes and documentation to meet standards.
