Zusammenfassung

Gateways Geelong is seeking experienced Practice Coaches to provide leadership, coaching and capability development across Community Services programs, including Social Connect, Recreation and Individual Supports.

You will translate participant goals into support plans, coach frontline staff and contribute to quality improvement, safeguarding and risk management to ensure safe, outcomes-focused supports.

Qualifikationen

• Certificate IV in Disability or related qualification.
• 2–3 years’ experience supporting people with disability and improving outcomes.
• Experience coaching or providing practice support to frontline staff.

Aufgaben

• Provide specialist practice leadership, coaching and capability development across Community Services programs.
• Translate participant goals and support plans into safe, effective services.
• Guide on complex participant needs, reflectively review cases and support quality improvement.
• Contribute to safeguarding, incident management and risk controls.
• Support safe delivery of High Intensity Supports and collaborate with participants, families, staff and stakeholders.
