Job summary

Y Whittlesea is seeking a part-time Support Coordinator based in Horsham or surrounding areas to help individuals navigate NDIS supports and access services.

You'll work in a Centre for Participation, coordinating plans, setting goals, monitoring progress, and solving barriers in a strengths-based, supportive environment.

This role offers flexible hours (0.6 FTE) and salary packaging, with potential to increase hours as your caseload grows.

Qualifications

• Strong support coordination and disability sector experience (12+ months highly regarded but not essential).
• Experience navigating NDIS legislation.
• Certificate IV in Community Services or equivalent.
• Current Working with Children Check.
• NDIS Worker Screening Check.
• Current First Aid and CPR (or able to obtain).
• Current drivers licence and access to a reliable car.

Responsibilities

• Service Coordination: Assist clients in implementing their NDIS plans and connect them with appropriate services, providers, and resources.
• Goal setting: Work collaboratively with clients to identify their personal goals and create action plans that support their progress.
• Monitoring and reviewing: regularly assess client progress, adjust plans as needed, and ensure services are meeting their needs.
• Problem-solving: Address any issues or barriers that may arise, ensuring clients have the support they need to thrive.
