Job summary

OnSide Support Coordination in Frankston is seeking a dedicated Support Coordinator to help clients navigate their NDIS plans and access community services. This permanent role offers flexible work patterns, with 1–2 days per week from home, 1–2 days visiting clients, and 1 day at an office location to collaborate with the team.

You will coordinate supports across providers, build client capacity, and monitor funding budgets, assisting people of different ages to achieve their goals.

Qualifications

• NDIS Worker Screening Check clearance.
• Current driver’s licence and insured vehicle.
• Eligible to work in Australia.
• Completed the NDIS Worker Orientation training: Quality, Safety and You.

Responsibilities

• Support clients to build capacity and understand the NDIS.
• Coordinate supports across multiple providers.
• Plan coordination to enable greater independence across life domains.
• Work within funding levels and monitor budgets for each client.
• Gather and document information for evidence-based reports and updates.
• Liaise with key stakeholders to coordinate service delivery toward NDIS goals.
