Job summary

YMCA Victoria in Horsham is seeking a part-time Support Coordinator to assist people with disabilities in accessing services and living independently. You will implement NDIS plans, connect with providers, and support goal achievement in a person-centred manner.

Ideal candidates have 12+ months in disability support, knowledge of NDIS legislation, and a Certificate IV in Community Services or equivalent; WWCC and a valid driver’s license are required.

Qualifications

• Strong support coordination and disability sector experience (12+ months helpful)
• Experience navigating NDIS legislation
• Certificate IV in Community Services or equivalent, WWCC and first aid where applicable
• NDIS Worker Screening Check and current driver license are required, with reliable vehicle access

Responsibilities

• Assist clients in implementing their NDIS plans and connect them with services, providers and resources.
• Collaborate with clients to identify goals and create action plans.
• Regularly monitor progress, adjust plans, and ensure services meet needs.
• Address issues or barriers to ensure clients receive needed support.
